Romance, fashion, and cross dressing. I am "old fashioned", so take this with a grain of salt, but it does kind of make me wonder at what point culture dictates what we consider acceptable. Jesus wore robes. The Scottish wore kilts in World War I (I found that out in Among the Poppies). So when did it change? Is it an American thing, Western thing, what? Would the book have the same issue if there was a large young lady who chose to wear clothing that tends to be more acceptable for those of a more slender stature? I don't know, but really, the essence of the book is how much do we accept a person despite his/her crazy ideas?
